Notice on S1 Discretionary Places Application

In accordance with the S1 Discretionary Places Application Circular issued by the Education Bureau, our School’s application procedures are as follows:
Period: 2 January 2009 to 19 January 2009 inclusive
Office Hours: 9.00 a.m. to 5.30 p.m. for Monday to Friday
9.00 a.m. to 3.30 p.m. for Saturday
No. of Places: 57
Procedures: Within the application period, download the Notice on S1 Discretionary Places Application and the Application Form for S1 Admission of our School from this webpage, or collect the relevant documents in person or by mail with a self-addressed envelope.
Notes:
The completed Application Form for S1 Admission should be submitted in person together with the following documents attached in the sequence as specified. All photocopies should be black and white and in A4 size. The recommendation letter from the primary school is not required when submitting an application. If the applicant fails to supply the necessary documents of items 1 and 2, the application will not be processed:
1. Show the original HKID card of the applicant to the office staff for verification.
2. In the Application Form for S1 Discretionary Place issued by the Education Bureau, fill in “CHEUNG CHUK SHAN COLLEGE” in the space provided for the “Name of School Applied For” in every copy and to be completed by the parent.
3. Fill in the Application Form for S1 Admission of our School.
4. Submit photocopies of P5 Annual/Final School Report and/or P6 First Term School Report (if applicable and in subsequent submission).
5. Submit photocopies of documents related to extra-curricular activities and services within the period from 1 September 2006 to 31 December 2008. If the activities or services have already been recorded in the school reports, photocopies of the relevant school reports can then be submitted in place of those of the activities or services concerned.
6. Submit photocopies of certificates or photos of prizes won at school or in open competitions within the period from 1 September 2006 to 31 December 2008. If the awards have already been recorded in the school reports, photocopies of the relevant school reports can then be submitted in place of those of the awards concerned.
Criteria: Successful applicants will be selected according to the following admission criteria and weightings: Rank Order of the applicant provided by the Education Bureau (50%), Conduct Grade of the most recently issued school report (10%), extra-curricular activities & services (20%) and prizes won at school or in open competitions (20%), both within the period from 1 September 2006 to 31 December 2008. No interview is required.
Remarks:
1. Our School was formally approved by the then Education Department to continue using English as the medium of instruction on 1 December 1997.
2. In accordance with the regulation of the Education Bureau, each student may apply to not more than TWO secondary schools listed in the Handbook for Application for S1 Discretionary Places; otherwise, his/her chance of acquiring a discretionary place will be forfeited. For enquiries on the Secondary School Places Allocation (SSPA) System, please contact the SSPA Section of the Education Bureau.
3. All copies of the Application Form for S1 Discretionary Place will be acknowledged with our School Code, School Name and Chop. The parent should properly keep the Parent’s Copy for future reference. All applications, once submitted, cannot be withdrawn or cancelled.
4. The announcement of successful S1 Discretionary Places applicants will only be made by the Education Bureau together with the Central Allocation Results on 7 July 2009. Parents need not inquire the results from the school.
5. All unsuccessful S1 Discretionary Places applicants of our School will be automatically placed on the waiting list for S1 admission should vacancies arise. Applicants or parents need not contact the school in due course. Students not applying to our School for S1 Discretionary Places will not be considered when filling these vacancies.
6. The personal data provided by the parents in the Application Form for S1 Admission will only be used by our School for processing the relevant application.
7. A Parent Talk on "Strategies on S1 Discretionary Places Application" will be given in the School Hall at 2.30 p.m. on 13 December 2008. Before the talk from 1.00 p.m. to 2.15 p.m., all parents and students are welcome to visit our campus.
8. The Open Day of our School will be held on 15 February 2008 (Sunday) from 9.30 a.m. to 3.00 p.m. All students and parents are welcome.
